Practice profile
Location The Practice is sited amid the council built, but now partly privately owned housing estate of East Swindon. Swindon is situated in a beautiful part of the country with the Wiltshire Downs to the South and Cotswold country to the North. There are good road and rail links with easy access to the M4 and M5. History The practice was started in the 1950s as a single handed practice and has flourished to the five partner training practice we are today. We have been on the current site since 1960s and in the current building since 1991. We have been a training practice since the 1960s. Communication We have good internal communication with regular practice meetings twice a week. There is a practice education programme and we hold regular significant event analysis meetings. Appointment System The practice runs an appointment only system with built-in flexibility for urgent problems. The duty doctor for the day has no prebooked appointments and is free to deal with emergencies and urgent problems, and can see patients who feel they do not want to wait to see their usual doctor. Out Of Hours Work Out of hours medical care is provided by Swindon PCT based at the Great Western Hospital (Clover Centre). Practice Area We have a defined practice area in which we will register patients. This allows us to provide an efficient service to our patients within the local area. Clinics Diabetes & Asthma There are sessions dedicated to diabetes and asthma when patients with these conditions are seen by the Nurse followed by their own doctor. Child Health Surveillance Clinic Friday afternoons with the Health Visitors and Partners in rotation. Antenatal Clinics We work closely with our attached midwife providing maternity services. Immunisation The nurses run an Immunisation Clinic on a Wednesday morning. Smoke stop Nurse health educator provides support and advice in a structured anti smoking programme. In addition we provide Family Planning Services and fit coils. We are well equipped for doing Minor Surgery, undertaking a wide range of procedures. We have a trained nurse who visits housebound patients when necessary and runs our smokestop programme. Primary Health Care Team We have a 'core' Primary Health Care Team consisting of people actually working with us from the building. Together with an extended team including Psychology and a Dietician who work from the practice Out of Practice Commitments
Chemists We have a pharmacy on site. There are two other chemists close to the surgery which both offer a collection and delivery service to patients who are housebound. We work closely with the local Pharmacists. Training Jonathan Elliman and Philip Mayes are the trainers and have weekly protected tutorial time with the registrar. However, all partners take an active role in training and another weekly tutorial takes place with each of them in rotation. Attendance at the weekly day release course (at Swindon and Bath Postgraduate Medical Centres) is expected. From time to time we have medical students from Oxford and Bristol Universities attached to the Practice. We have a well organised practice library and full NHSnet and Internet links on all the surgery computers. We also have a video camera/TV/VCR, to help us examine our consultation skills, overhead projector and computerised presentation equipment are available. Information Technology The practice makes full use of the excellent EMIS computer system. We are working to be paperless, and record all consultations with major diagnoses and all medication on the computer. All our appointments are on computer. We are "linked" to the local hospital for electronic results and electronic out patient and fast track referrals. We also use Mentor the on-line textbook and Pils patient information leaflets and have full NHSnet access. We try to use “Choose and Book” software QOF We have achieved high QOF scores in the last couple of years PCT We are members of the Swindon PCT and are part of the East Swindon PBC group. We are a PMS Local Hospitals We have the facilities of the modern Great Western Hospital with most specialities, including Accident and Emergency located there. Other local hospitals are the Victoria Hospital (elderly psychiatry), Prospect Hospice (palliative care), Sandalwood Court (psychiatry), Ridgeway Hospital (private). Future Hopes We hope to develop the services that we offer to ill people, as well as to improve prevention of illness.
