ELM TREE SURGERY – High Street, Shrivenham, Swindon SN6 8AG
The practice is centred on the villages of Shrivenham and Watchfield, just off the A420 on the Oxfordshire/Wiltshire border. Swindon is 7 miles away and Oxford 24. The area has been traditionally agricultural but has become predominantly executive middle class. London is within commuting distance, Swindon is a focus for new high tech industry, and the UK Defence Academy in the village is now a university. The area is nevertheless still predominantly rural in atmosphere. Our hospital needs are usually met by the Great Western Hospital (GWH) in Swindon, or the various hospitals in Oxford. Private patients may be referred to the Ridgeway Hospital at Wroughton – located just outside Swindon. The Premises were built on the site of the pre-existing surgery in 1984, were soon enlarged and now provide six consulting rooms and a treatment room/nurse consulting room. There is also a large common room/meeting room and two offices. The practice is responsible for the care of the military officers and students at the Defence Academy. We hold a surgery there every day. Families and dependants of the military personnel usually register with us, providing us with a relatively high turnover of patients. Many families are from overseas, and we may have patients from over 40 different countries at any time. We are part of the Swindon Primary Care Trust (PCT). The list size is 6,200, 50% of whom are dispensing patients. Out of hours care is provided by Swindon PCT. There are 2 trainers in the practice – Dr Tony Crockett and Dr Margaret Andrews and usually two registrars – one of whom may be from the army.
The Training There is a 2-hour tutorial each week with the trainer (or another partner if he/she is away). There are joint surgeries, and a 2 week induction period when the registrar has the opportunity of working with each member of the Primary Health Care Team. All the partners are involved in training. The registrar has their own room, with access to a video camera. Training is learner-centred, with a flexible educational programme to suit the individual registrar. Day release (all day Wednesday) is on the Swindon/Bath scheme and there is a half day off each week. We are fully aware of the requirements of summative assessment, and of the MRCGP examination.
Partners
Dr Tony Crockett trained at Southampton and joined the practice in 1987 from his Wessex training practice. He is a hospital practitioner in the Chest Clinic at GWH and has a particular interest in asthma, and has written two textbooks and numerous articles about asthma and COPD. He is a FRCGP. Dr Margaret Andrews trained in London and joined the practice as a partner in 1994, having previously been our trainee. She has extensive medical experience including the MRCP (UK) and Diploma in Occupational Medicine, and is a hospital practitioner at GWH. Dr Richard Fisher trained in Bristol and joined the practice in 1995, having also previously been our trainee. He has extensive medical experience especially in ophthalmology, for which he is a hospital practitioner at GWH. He is the student tutor for the practice. Dr Sian Edwards joined in 2003, and was also a registrar here and in High Wycombe. She has particular interests in Women’s health and paediatrics.
Ancillary Staff There are three practice nurses and a health care assistant, seven receptionists, three secretaries, a filing clerk and a Practice Manager. Attached Staff We have three district nurses and two health visitors attached to the practice. The midwife, occupational therapist, social worker and community psychiatric nurse liaise with us closely. We have a physiotherapist who attends twice a week, clinical psychologists who holds weekly clinics and a dietician twice monthly. Practice Organisation All patients are seen by appointment. Emergency appointments are made at the next available opportunity. We aim not to defer work until the next day if possible but appointments for a specific doctor may only be possible a day or two ahead. We do not run a personal list system. Patients are free to see the doctor of their choice irrespective of their registration. We encourage patients to stay with one doctor for a particular illness. All notes are summarized. The computer is used for prescribing and for consultations. We have electronic links with laboratories and X-ray departments at the GWH, Swindon PCT and with Thames Valley and Wiltshire Health Authorities. Minor surgery and trauma are dealt with on the premises where possible.
Special Clinics Antenatal, most contraception work, health promotion, chronic disease and minor surgery are all done during normal surgeries.
Audit Activities 1. A practice formulary has been in use for several years with the aim of harmonising and rationalising prescribing habits 2. Protocols for the management of hypertension, diabetes and asthma have helped the practice nurses play an important role in the management of these diseases 3. We have frameworks for the nurse prescribing and treatment room duties 4. We usually achieve our cytology and immunisation targets. Details of recent audits are available 5. Equipment Minor ops; resuscitating; ECG; defibrillator; sonic-aids; audiometer; teaching auroscope; nebulisers; electro-cautery; tympanogram; fax machine; Doppler; spriometer. There is an indexed library and journal collection. An overhead projector and a PC based projector enhance our practice educational activities. We are fully computerised (EMIS), and have internet/email access as well as our own website.
The Area and its Amenities Shrivenham is a pleasant village set in open countryside near the Ridgeway and the Marlborough Downs and not far from the Cotswolds. The village contains basic shops, a bank, post office, two churches, three restaurants, three pubs and a primary school. In the village there are tennis, cricket, football, gliding, golf and bowls clubs, whilst at the Defence Academy there are squash, tennis and golf facilities and a swimming pool. Swindon is 7 miles away, where there are swimming pools, ice-skating, hang-gliding and microlite facilities. There are water sports at the nearby Cotswold Water Park. There are theatres, concert halls, cinemas and extensive shopping facilities in Swindon and Oxford and in nearby Bath, Cheltenham and Newbury. Oxford is 24 miles away and has fine museums, theatre and concert halls and a host of cultural events.
